  • Diabetes care
    • Diabetes Center: The Diabetes Center at Atrium Health Cleveland includes nurses and registered dietitians who will help guide you through the process of minimizing the impact of diabetes and help you can get back to better health. For more information, please call 980-486-3953.
  • Orthopedics
    • Additionally, the Joint Academy is a specially designated unit at Atrium Health Cleveland for joint replacement surgery patients, including hip, knee and total shoulder replacement. The Joint Academy is designed to shorten recovery time and help patients return to normal activities as quickly as possible.
    • Patients are referred to the Joint Academy program by their orthopedic surgeon. The surgeon bases his recommendation on how much the patient will benefit from the program and how well he thinks the patient can handle the fast pace and comprehensive treatment.
    • From extensive pre-operative education and exercise to post-discharge care, we coordinate your care, working toward your quickest and safest return to normal activities.
    • Patients of The Joint Academy have surgery on Monday and are out of bed day of surgery to begin therapy.
    • The Joint Academy staff – including registered nurses and certified physical therapists – work with each patient to ensure they have a successful stay and are ready for graduation and discharge.
    • Graduates of the academy range in age from 30 to 80.
  • Care Solutions offers programs to help patients and caregivers find the care and services they need, including:   
    • Community Care Management: This program focuses on identifying the needs, coordinating care and locating resources for adults age 18 or older. Individuals may be self-referred or referred by the community, hospital case managers, neighbors and churches. 
    • Respite Program: Caregivers who need to find time for themselves can turn to this program for adult day healthcare or in-home aide. The services are available to caregivers who are age 60 or older, or caregivers who are under the age of 60 who care for someone age 60 or older.
    • Community Alternatives Program for Disabled Adults: This program helps Cleveland County residents who are eligible for Medicaid and at risk for nursing home placement. We offer services that allow adults 18 or older to stay in their home and community.
    • Private Care Management: This private-pay program provides a nurse and a social worker to meet with patients and caregivers to identify needs, locate resources and develop a personalized care plan. The program is available to adults 18 or older.
