Which doctors practice at Proactive Health?

Melanie McMinn, MD and Greg Weidner, MD. Together they have nearly 30 years of experience in adult Internal Medicine and are both highly rated for quality and patient satisfaction. Dr. McMinn holds additional certifications in Culinary Medicine, is a certified Hypertension Clinician, and has received additional training in Mindfulness. Dr. Weidner is a Fellow of the American College of Physicians and is a national leader in digital health and patient engagement. We plan to add another physician within the first year, carefully selected to meet our high standards.

Is Proactive Health part of Carolinas HealthCare System?

Yes. While we have developed a new model for primary care, we are preserving the many benefits that come with being part of a leading healthcare system. These include access to world-class specialty and hospital care, electronic medical records, and the My Carolinas patient portal.

Is Proactive Health right for me? I'm not sure based on my age, medical conditions and health goals.

Since Proactive Health individualizes your health and medical care, we believe that just about everyone can benefit from our enhanced connected care approach. Our team can help you make the most of our practice offerings and tailor your Proactive Health experience to meet your health goals. Proactive Health sees patients who are at least 18 years old, and there is no upper age limit.

How much is the Proactive Health subscription?

If paid monthly, the subscription cost is $75 per month. We offer a 5% discount if you choose to pay annually, for a total of $855 per year.

Is there an enrollment fee?

Yes. The one-time enrollment fee is $100. This fee will be waived for prior patients of Drs. McMinn and Weidner.

What does my subscription fee cover?

Your subscription covers features that are not covered by health insurance plans. Through a connected care approach, we partner with you to help you be your healthiest and happiest self. Your subscription covers a well-designed suite of convenient technology channels that connect you to your health, healthcare team and the Proactive Health community:

  • Collaborative Care Platform: This hassle-free mobile and/or web-based technology solution allows you to set goals and track progress as well as interact conveniently with your health advocate, dietitian and care team.
  • Virtual Touch Points: Our technology platforms support video or phone visits with your team. Our "Family and Friends" offering also allows requested family members to participate in your office visits even if they can’t be present in person.
  • Health Advocates: We partner with you to develop specific personal health goals, set action plans, and provide continuous support, guidance and accountability to enhance your success.
  • Nutrition Support: Our registered dietitian provides technology-enabled advice and support for lifestyle medicine and personal health goals, delivered through convenient technology channels that fit your life.
  • Proactive Health Chat: Using our innovative Proactive Health chat function, we provide a comprehensive health assessment and then periodic digital checkups to help you identify and manage health concerns with the help of your Proactive Health team.
  • My Proactive Health Pulse: Based on the personalized health assessment of your current medical conditions, health habits and risks, we partner with you to create your My Proactive Health Pulse – a personalized health action plan.
  • Onsite Proactive Health Hub: Our onsite digital bar provides a space for you to test and experience current mobile apps, remote diagnostic devices, and trackers/wearables to help you stay on top of your health.
  • Proactive Health Community Events: We offer events on relevant health topics, like cooking demonstrations, mindfulness classes, digital health tutorials and more. These give you an opportunity to connect, engage and socialize with your care team and others focused on improving total health. These can happen in person or by remote technology for your convenience.
  • Proactive Health Online Community: Our professionally moderated online community gives you a safe and helpful place for support, education, digital resources and peer-to-peer interaction.
  • Healthy Living Resources: We provide content created and curated to support your health. Resources include a lending library, YouTube channel, digital resources, personalized recommendations for fitness centers and more.

Your experience will be tailored to fit your needs. Through our connected care approach, we empower and encourage you to improve your health for your future. We strive to remain on the cutting edge of advances in personalized, preventive care, and continually add new solutions and products to benefit our patients.

How does Proactive Health work with insurance?

Your subscription fee covers features that are not covered by health insurance plans. Your insurance will still be charged to cover any healthcare visits and services unrelated to your wellness program – including office visits, specialist visits, hospital stays, lab and diagnostic tests. Copayments, coinsurance and deductibles will apply as usual.

Will my insurance still be accepted for billable healthcare expenses?

Proactive Health participates in most plans accepted by Carolinas HealthCare System. Our team can help answer any specific individual questions you may have.

Will Medicare be accepted?

Yes. Your Medicare Part B coverage will continue to work for services that are currently covered. Your Proactive Health subscription fee covers those services not reimbursed by Medicare, as listed above under “What does my subscription fee cover?” Our team can help answer any specific individual questions you may have.

My insurance covers an annual physical. How does Proactive Health subscription complement my physical exam?

Your Proactive Health team will ensure that you receive all recommended preventive services, including an annual wellness exam. We will take it further with a personalized action plan (your My Proactive Health Pulse) – which we will co-create with you and then provide you support, encouragement and accountability throughout the year as you pursue your goals.

Will being a Proactive Health subscriber save me time and/or money?

We understand that people are seeking value – the highest quality care and health services delivered in the most cost-effective way. Our practice is built to align our incentives with yours in order to maximize value. Your connected care team will work together to be certain diagnostic tests and referrals are done in the most cost-effective way. Your subscription also includes health services that you might pay extra for elsewhere. We promise to value your time and money as highly as we value your health.

Can I use my HSA (Health Savings Account) to pay for Proactive Health?

Proactive Health fees cover services which can potentially be eligible for HSA reimbursement for many individuals. We will provide a statement of services delivered upon request for HSA purposes.

Can I purchase a Proactive Health subscription for someone else?

Yes. We can help you purchase a subscription for family members and other loved ones.

What if my circumstances change or I am not satisfied? Can I get a refund?

Our goal is 100% satisfaction with the Proactive Health experience. You may cancel any time after 90 days if you are not completely satisfied or if your circumstances have changed, and we will happily provide a prorated subscription refund.
