BY ACCESSING OR USING THE SITE, YOU AGREE TO BE BOUND BY THE TERMS AND CONDITIONS OF THIS AGREEMENT. PLEASE READ THEM CAREFULLY. WE MAY MODIFY THIS AGREEMENT AT ANY TIME, AND SUCH MODIFICATION SHALL BE EFFECTIVE UPON POSTING.
UNAUTHORIZED ACCESS IS STRICTLY PROHIBITED
1.a. Educational Content; No Medical Advice.
The informational content contained on the MyCarolinas home page and in the sections on help and resources is presented for the purpose of educating consumers on wellness and disease management topics and is not intended to be used for medical diagnosis or treatment. The information should not be considered complete or timely, nor should it be relied on to suggest a course of treatment for a particular individual. In many situations, you may need to talk to your healthcare Provider to interpret what you are seeing. If you have questions, please contact him or her.
ALWAYS SEEK THE ADVICE OF YOUR PHYSICIAN OR OTHER QUALIFIED HEALTH PROVIDER WITH ANY QUESTIONS YOU MAY HAVE REGARDING A MEDICAL CONDITION. NEVER DISREGARD MEDICAL ADVICE OR DELAY IN SEEKING IT BECAUSE OF SOMETHING YOU HAVE READ ON THIS SITE.
1.b. Health Records; Appointments and Billing; MyProfile.
The information contained in the Health Records, Appointments and Billing and MyProfile sections of MyCarolinas is compiled from a variety of sources ("Content Providers") and is to be used by you to access your health records and for the management of medical bills and scheduling healthcare services. Neither we nor our Content Providers directly or indirectly practice medicine or dispense medical services as a part of these functions. You acknowledge that you are aware that the information collected may be accessed by your Carolinas HealthCare System provider, it agents, or other designated persons as Carolinas HealthCare System deems necessary and appropriate.
We will not provide you with your entire medical record on this Site. If you want a complete copy of your medical record, please contact our Health Information Management department.
By using this Site and giving us your email address, you consent to us sending information to you about your health or reminders, including for appointments or follow up, from the portal to your personal email address. You may wish to review the Guidelines for Email with Patients available on our Site, www.carolinashealthcare.org under the Privacy link, for more information about the risks and benefits of using a personal email address. Please note that we will never send you an email asking for your confidential password. Call us immediately if you receive such an email.
This Site is monitored on a periodic basis, but it is not monitored 24 hours a day. Please do not rely on it to communicate with your provider, particularly if you have an urgent medical situation. IN AN EMERGENCY, CALL 911.
Health Messaging and eVisit are to be used for non-urgent, non-life threatening communication. If you have an urgent or life threatening issue, call 911 or go to the nearest emergency room. Your decision to communicate with a provider is voluntary and does not affect your ability to contact your facility directly to speak with your healthcare provider and use the medical services available in the traditional environment (such as in the physical presence of a provider or in a hospital setting). Secure messages may be screened by administrative staff before being forwarded to a healthcare provider. Health Messaging and eVisit is intended to supplement and not replace your existing primary care physician relationship.
Health Messaging and eVisit are provided over a secure connection. Secure messaging is not email. No part of the actual secure message will be transmitted via email. Security measures are in place to protect the privacy of the information transmitted. You are responsible for maintaining the confidentiality of your account and password, and for restricting access to your device. It is your responsibility to ensure that your personal information is not compromised due to the lack of secure equipment and/or surroundings.
You are responsible for logging out of eVisit or Health Messaging when your session is finished and for closing the browser being used to access these services. If you print any secure messages, you are responsible for protecting that information.
eVisit and Health Messaging communications may become a part of your confidential health record. You have the right to access your medical record and see this information, subject to applicable privacy and medical records policies
eVisit Consent to Treatment.
eVisit services are intended to provide an easily accessible means of obtaining healthcare information about a defined set of common medical conditions. eVisit services are only available to established patients of Carolinas HealthCare System and eVisit healthcare providers will access your Electronic Health Record to obtain information regarding your treatment history. You should promptly contact your primary care physician if you have questions, difficulties, or complications in connection with any treatment recommendations. You can and should share any treatment recommended through eVisit with your primary care physician. There are no guarantees as to how your treatment will proceed or turn out as a result of your exchange with a provider by eVisit. The provider may decide that it is in your best interests to be seen in person.
By using the eVisit service you consent to the provision of treatment through eVisit and you understand the risks and benefits of using eVisit. Services provided by eVisit may include consultation, diagnosis, referral to another healthcare provider and/or recommendation for a different level of care as necessary. In submitting a request for an eVisit, you represent that the following statements are true:
You will provide truthful, accurate and complete information to the eVisit healthcare provider, and acknowledge that any failure to do so could result in a wrong diagnosis or treatment decision that could be harmful or ineffective. Accordingly, you have responded to each question as part of eVisit truthfully and accurately and have fully and completely disclosed any and all information concerning your health and medical history that you are aware of which could be relevant to your current condition and need for treatment and/or medication.
You understand that the eVisit healthcare provider will only be able to diagnose and recommend a treatment for your condition based upon information that you have provided and any relevant information provided by your medical history as documented in your medical record. If you fail to provide information that is required to complete a visit, Carolinas HealthCare System or the eVisit healthcare provider reserves the right to terminate any eVisit.
You agree to fully comply with all instructions provided by the eVisit healthcare provider following the submission of answers to questions required to obtain the diagnosis/treatment received. Carolinas HealthCare System is not liable for your failure to adhere to or be compliant with the healthcare provider's recommendations, which may include a recommendation for an in-person visit with a provider or specialist rather than a diagnosis and recommended treatment. You will have the opportunity to ask questions about any diagnosis, treatment options, and/or referrals provided for you through the eVisit service, and it is your responsibility to ask such questions. To participate in eVisit you must be in North Carolina at the time of the service and all online consultations, diagnosis and treatment will be deemed to have occurred in North Carolina.

MyCarolinas Tracker is not a substitute for medical advice or attention, and is not designed to be used in case of a medical emergency.
