Make an Appointment at Suburban Pediatrics

For routine or follow-up visits to Suburban Pediatrics, always bring your insurance cards, including Medicaid cards and all prescription medications.

First Visit

Each new patient is asked to complete a health history form before their first visit to Suburban Pediatrics. This will help our medical staff make a prompt and accurate assessment during the first visit. Please bring a copy of your child's current medical records, or ask your previous office to send the records to us prior to your visit. Please bring your current insurance cards, including Medicaid, current immunization records and any current prescriptions.

Please arrive 20 minutes before your scheduled appointment to complete registration and other paperwork, or print and complete the online forms before your arrival.

Cancellation Policy

Please call to cancel your appointment at least 48 hours in advance if you will be unable to keep it so we may offer your appointment to other patients needing treatment.

Emergency and After Hours Care

Our physicians are available for emergency care. Please call our office before going to the emergency room. If you are in need of after-hours urgent/emergency care, call the Kannapolis office number at 704- 403-7740. Please tell our after-hours service that your situation is urgent. However, if it is a true life-threatening emergency, please go to the emergency room.

Please remember that the doctor is available after hours only for medical emergencies. Most medical problems can be addressed during our extended regular office hours. No refills of prescriptions will be called in after hours or on weekends.

Prescriptions and Refills

Please bring all prescription medications to each appointment, even if prescribed by another physician. Whenever possible, please ask for all refills of continuous medications during your appointment.

If your prescription expires before your next visit, please contact our office for refills. Please allow 48 hours to process refill requests. If this request is received on a Friday, Saturday, Sunday or a holiday it may be processed the following business day.
