PATIENT INFORMATION
Medication refills
To obtain a medication refill by telephone, patients should call the office during regular office hours.
Calls are usually returned either in the late morning or late afternoon. Controlled drugs will not be
refilled after regular office hours or on the weekend.
Emergencies
A physician is available 24 hours a day to assist reproductive endocrine patients with emergencies. To
reach a physician, those patients should call 704-355-3149. All other patients should contact their
primary care physicians for after-hour emergencies.
Medical forms
All new patients will receive an information packet that should be completed prior to the first appointment. If past medical records are requested, patients should be sure these are sent to CMC's Women's Institute office several days prior to the scheduled appointment.

For the protection of the patients, CMC's Women's Institute will not release any medical information
without the patient's written authorization, unless required to do so by law. When the patient signs a
release form, they must understand that CMC's Women's Institute cannot withhold any medical facts from the
persons or organizations for which release of information has been authorized.
Insurance and Fees
CMC's Women's Institute will be happy to bill the insurance company for all covered services. However,
because the health insurance is a contract between the insurance company and the patient and not between
the insurance company and the physician, the patient is ultimately responsible for prompt payment of all
account balances.
If the insurance carrier requires pre-authorization, the patient will need to obtain this prior to
their scheduled appointment. CMC's Women's Institute will assist the patient with any problems they may have
regarding insurance claims, once they provide the correct information. If you have questions about insurance
