HOME INFUSION AND EQUIPMENT

A Team Approach to Delivering Advanced Technological Services In the Home:

CMC Home Infusion and Equipment is committed to providing excellent service with quality care and compassion. We provide a broad range of infusion services for patients of all ages to be administered in the comfort of their own homes. Medical Equipment, such as the LVAD appliance for Cardiac patients and CPAPs for patients with sleep related illnesses, can be provided. Oxygen services, along with other respiratory equipment, will be added in the summer of 2006. Our Registered Respiratory Therapists work with each patient to secure their commitment to full compliance, thus striving for the best outcome in patient satisfaction and improvement. Decreasing hospitalizations, infections and healthcare crises are the ultimate goals of CMC Home Infusion and Equipment.

The CMC-Home Infusion and Equipment Team is represented by:

  • A Home Care Coordinator, who takes referrals and coordinates all home care services required to support the patient and his or her primary caregiver
  • An Intake Specialist who works with the patient's insurance company to establish authorization for the services
  • A Clinically Trained Pharmacist who follows the patient from admission to discharge. Prescription adjustments and clinical communications with the physician, nursing agency and pharmacy, along with patient education are all critical parts of the pharmacist¹s primary role.
  • A Pharmacy Technician who assists the pharmacist in preparing the prescriptions and supplies for delivery to the patient
  • A Registered Nurse who educates and supports the patient and his caregiver in the safe administration of the home infusion therapy until he feels fully informed and confident and is able to administer the appropriate therapy without supervision. Both the nurse and pharmacist are available to the patient 24 hours a day for questions and/or concerns.
  • A Billing Specialist who is dedicated to working with the patient's insurance company to expedite reimbursement for services rendered and avoid patient involvement if at all possible

Infusion Therapy:

  • Total parenteral and enteral nutrition
  • Inotropic therapy
  • Anti-infectives, anti-virals, anti-fungals
  • Pain management
  • Chemotherapy
  • IVIG
  • Chelation therapy
  • Biological medications
  • Enzyme deficiencies

Home Medical Equipment including but not limited to:

  • Oxygen and other respiratory equipment
  • Sleep apnea equipment and services
  • LVAD
