Patient Information

Consent for Treatment

A parent or legal guardian should accompany the child to all visits. North Carolina law requires parental permission prior to the treatment of any child under the age of 18, except under certain circumstances. If you cannot accompany your child, please send a written statement giving permission to treat your child.

Appointment Information


If your child is sick and his or her condition requires a same-day appointment, the pediatrician who is immediately available will see your child. You may, however, specify a particular pediatrician when scheduling well-child check-ups.  Routine appointments can be scheduled from 8 a.m. to 5 p.m., Monday through Friday. For after-hours emergencies, call our office and the answering service will page the physician on call.

First Visit

New pediatric patients must complete health history form before their first visit to Rocky River Pediatrics. This will help our medical staff make a prompt and accurate assessment of your child during the first visit. Please bring your current insurance cards, including Medicaid cards, and any current prescription bottles. Please arrive 20 minutes before your scheduled appointment to complete registration and other paperwork.

Routine Visit

The goal of Rocky River Pediatrics is to provide children with prompt, easy access to quality healthcare. Our medical staff is available to see patients by appointment during normal office hours. Please arrive 15 minutes before your scheduled appointment.

Urgent Sick Visits

Rocky River Pediatrics provides evening and weekend hours for urgent visits by appointment in our office.

Cancellation Policy

Please call to cancel any appointments you are unable to keep at least 24 hours in advance so we may offer these appointments to other patients in need of treatment.

Billing and Insurance

Rocky River Pediatrics accepts most major insurance carriers and will assist you in filing claims for healthcare services. Should you have any questions about any insurance or billing issues, please call our office during normal business hours.


Please bring your insurance card with you to each visit so we can file your insurance claims properly. Co-payments, deductibles and co-insurance amounts are due at the time of service. We accept cash, check, Visa, MasterCard and Discover.

When your insurance has responded, any remaining balance will be billed to you at your home address. Please remit any payment due within 30 days. If you are unable to pay at the time of your visit, you can discuss payment options with Carolinas HealthCare System's central billing office at 704-512-4808.
