Piedmont Orthopedic Specialists is pleased to provide our patients with some of our most frequently used forms. For your convenience, you can download and print the forms at home, then fill out the information and bring it with you on your next visit.

Appointment Information

First Visit

Although a physician referral is not required, some insurance companies require a referral from your primary care physician. Our office highly recommends that you are first seen by your primary care physician and referred to our office if needed. New appointments are typically scheduled through the primary care physician's office.

Prior to your first appointment, each new patient is asked to complete a series of forms. This will help our medical staff make a prompt and accurate assessment during the first visit. Please bring your current insurance cards, including Medicare cards as well as any current prescription bottles. Please arrive 20 minutes before your scheduled appointment to complete registration and other paperwork, or print and complete the online forms before your arrival to save time.

Follow-Up or Routine Visit

For a follow-up, post-surgery, or routine visit, please arrive 15 minutes before your scheduled appointment. Always bring your insurance cards, including Medicare cards and all prescription medications. Please notify the office staff of any changes to these items.

If surgery is recommended, our staff will be happy to handle all scheduling aspects of the patient's surgery, including insurance authorization.

Cancellation Policy

Please call to cancel any appointments you will be unable to keep at least 24 hours in advance so we may offer these appointments to other patients in need of treatment.

Piedmont Orthopedic Specialists accepts most major insurance carriers and will assist you in filing claims for health care services. Should you have any questions about any insurance or billing issues, please call our office during normal business hours.

Billing and Insurance Information

Fees

Co-payments, deductibles and co-insurance amounts are due at the time of service. We accept personal checks, Visa, MasterCard and Discover cards. When your insurance has responded, any remaining amount you owe will be billed to you at your home address. Please remit any payment due within 30 days. If you should need to make special arrangements concerning your bill, please call the office prior to your appointment.

Insurance

Some insurance companies require a referral from your primary care physician. If surgery is recommended, our staff will be happy to handle all scheduling aspects of the patient's surgery, including insurance authorization.

Medicare and most private insurance companies are accepted. Please remember to bring your insurance cards to each visit and notify us of any changes so we may file your insurance properly.

Prescriptions

Please bring all current medications, including any vitamins and supplements, to each appointment - even if prescribed by another physician. Whenever possible, please ask for all refills of continuous medications during your appointment.

If your prescription expires before your next visit, please call the pharmacy that filled the prescription so that they may contact our office for refills. Calls to pharmacies regarding refills are usually returned the same business day.
