New patients need to have a referring physician call to schedule their first appointment with the clinic. Once contacted by your physician, please call our office to confirm your appointment.
To make a returning appointment, please call 704-403-1308 and select prompt #1. In the event our staff is unable to answer your call, please leave a message and your call will be returned as soon as possible.
Each new patient is asked to print and complete the new patient forms.
Please bring your insurance card and any prescription medications to your appointment. Each new patient is asked to arrive 30 minutes early to complete registration and other paperwork. Returning patients are requested to arrive 15 minutes early so we may verify and update any information as needed.
Our physicians are on-call 24 hours a day to offer help to patients in need. If you have an urgent problem, after hours or on a holiday, please call CMC-NorthEast at 704-403-1308.
Please call to cancel any appointments you will be unable to keep at least 24 hours in advance so we may offer these appointments to other patients in need of treatment.
Billing & Insurance
NorthEast Rheumatology accepts most major insurance carriers and will assist you in filing claims for health care services. Should you have any questions about any insurance or billing issues, please call our office during normal business hours.
Co-payments and co-insurance amounts are due at the time of service. We accept cash, personal checks, Visa, MasterCard, Discover and American Express. When your insurance has responded, any remaining amount you owe will be billed to you at your home address. Please remit any payment due within 30 days. If you should need to make special arrangements concerning your bill, please call the office prior to your appointment.
Medicare, Medicaid, and most private insurance companies are accepted. Please remember to bring your insurance cards to each visit so we may file your insurance properly. For insurance providers with which we are not contracted, you will be given a copy of services and charges to file with your insurance claim form. However, as always, it is your responsibility to know your terms and coverage for your own insurance contract as providers are too numerous and varied for our staff to know all details of all available plans.
If your health insurance is provided through Medicaid, you will be asked to show your Medicaid Card at each visit. As you're aware, Medicaid eligibility is determined on a monthly basis. In some cases, the situation changes each month so it's important that we have updated and current information for each visit. If you have chosen a Carolina Access Primary Physician, you must have authorization to see a specialist.
Please review your medication supply regularly to ensure you do not run out. If your prescription runs out or expires before your next visit, please call the pharmacy that filled the prescription; they will contact our office for refills.
Controlled substances (pain medications) must be called into our office at 704-403-1308. You will be prompted to press 3 for refills. Please have all the following information when you call:
- Patient's name
- Patient's date of birth
- Patient's telephone number
- Name of medication
- Medication instructions
- Name of pharmacy
- Pharmacy telephone number
Routine refills are done from 8:30 a.m. to 3 p.m. All requests received after 3 p.m. will be handled the next business day. We cannot refill controlled substances after business hours.
Please allow 24 hours for medication refills.
We do not dispense prescriptions or sample medication between 12 and 1 p.m.
