 QUICK FACTS
Living Wills
Advanced medical technology sometimes creates complex ethical choices, such as deciding about life-sustaining treatment or interpreting a living will. As a result, you need to make your wishes known in advance about how you want your healthcare managed. Several forms are available that allow you to instruct healthcare providers about treatment you may or may not want. These forms are called Advance Care Documents. They include:

Living Will

Health Care Power of Attorney

Advance Instructions for Mental Health Treatment
For more information and/or to create your own Advance Care Documents, click here.
