Chaplains are available to visit patients and families offering prayer, a listening ear, and help in coping with the challenges of being sick and in the hospital. Chaplains can also offer information and assistance with advance care planning.

Advance care planning consists of four steps:

  1. Knowing basic choices about the use of life support.
  2. Clarifying those choices from a spiritual perspective and personal values.
  3. Discussing choices with loved ones.
  4. Documenting choices using a living will, a healthcare power of attorney, or a do not resuscitate order.

Carolinas HealthCare System Cleveland is a member of Journey, a partnership of local healthcare providers and service agencies. Journey works to educate residents about end-of-life issues and caregiver support.
