Medical Staff Services strives to provide credentialing services to the medical staff and allied health professionals in a professional and timely manner. Please complete in its entirety the Private Provider Information Form, this form is used to initiate the application process for private providers. Once you have completed the Private Provider Information Form, please email the completed form to MSSproviderREQ@carolinashealthcare.org
After Medical Staff Services has received your Provider Information Form you will receive an email from the Intake Coordinator requesting that you complete the Pre Application. You will need to enter the email address provided in the email, then click “First Time Login”. An email will be sent to you with a link. Follow the directions in the email to create your password. Please save this password, you will need it to complete your full medical staff application at a later date. Once completed, sign electronically and submit within 1 business day.
SPECIAL NOTE: Upon our review of your pre-application and it is determined you meet eligibility; you will be sent a second email with a link to your medical staff application.
To complete the medical staff application, you will need to access the link below. You will need to enter your email address. If you have not previously created a password, you will need to select “First Time Login” to create one.
Due to the time-sensitivity of this information and levels of approval, it is imperative that you submit your application along with all supporting documents expeditiously or within 5 days of receipt of this email to avoid potential delays in your scheduled start date. If we do not receive the application and all supporting documents within 10 days, your packet is considered incomplete and we will request a new start date.
Through this real-time portal the provider’s application status can be tracked, in addition to any outstanding documents that are needed to complete the process.
Providers are notified of their re-credentialing via the email address on file with the Medical Staff Services Office, and no longer receive a paper packet in the mail.
To access the NAMSS PASS™ visit NAMSS PASS External Icon. Login to the NAMSS Website using your NAMSS member username and password, or by creating a guest account. Once you log in, click on Access the NAMSS PASS.
You will have access to all affiliations that have been uploaded to the NAMSS PASS™ for each practitioner (including their affiliation with CHS and the dates of that affiliation. You will then have the option to print an affiliation letter indicating the status of good standing.
Requests to verify a GME resident's or fellow's training at Carolinas Healthcare Systems are processed by the individual program where the resident/fellow completed that training. Please contact programs directly with your verification inquiries; the Medical Staff Office does not complete verification of training forms. Please use the link to contact the program directly.
