Hospital Admissions

Unless you're an emergency patient, your physician will schedule you for admission at Carolinas HealthCare System Union. Pre-admission forms are available at your physician's office or the Carolinas HealthCare System Union Admitting Office. To better serve you, we try to contact you prior to your scheduled visit to obtain current demographic and insurance information. We also provide bedside registration for inpatient stays. Please provide us with your insurance information at this time if you did not pre-register for your hospital stay. You will also be asked if you have an advance directive, living will for healthcare, or healthcare power of attorney, stating your wishes concerning the making of healthcare decisions that affect you.

What to bring to the hospital:

  • Insurance forms and cards
  • A copy of your Advance Directive form
  • Money for any insurance co-payment or deductible that your plan requires
  • Personal toiletries
  • Pajamas or gown
  • Bathrobe and slippers

What to leave at home:

  • Valuables other than money that may be asked for co-payments or deductible (money, billfold/wallet, purse, credit cards, jewelry)
  • Electrically operated appliances
  • Medications (unless your doctor asks you to bring them)
