|What constitutes a properly labeled blood bank sample for a type and screen/ hold or a type and cross?|
|Per professional guidelines, a blood bank sample must be collected and labeled at the patient's bedside and include the patient's name, history number, specific blood bank number (armband number), the date, time and initials of the person who collected the sample. All of these must be handwritten (no addressographs). Specimens are to be collected in purple top tubes only, no bullets are accepted.
